Bundle the `nrf91-sm` Zephyr driver directly under `drivers/nrf91-sm/` instead of pulling it from the `serial-modem` west sibling repo. This removes the `serial-modem` west project dependency and pins the `nrf` revision to a specific commit. Doc updates pin the tested Serial Modem firmware commit and link to the official getting started guide for flashing. Signed-off-by: Simen S. Røstad <simen.rostad@nordicsemi.no>
